Morton Clinic News from Mrs. Skelly

I am in the building daily.  An authorized staff member will monitor the clinic in my absence. Please call me with any health changes or conditions that might impact your child’s performance in school. I can be reached at 440-257-5954.  

Emergency Medical Authorization 

The Board of Education requires an update of the Emergency Medical Form on the Parent Portal every year. This permits us to transport a child in case of an emergency and also give permission for your child to go out on a school event. To get to the portal, click here

General Information:

Forms from the Clinic - Click to view list. All medications to be administered at school require a Medication Administration form and  Parent Permission form.  Additionally, Care/Action Plans are needed for specific health conditions. Select the form you need to download and print.  

Illness/Injury at School:  Minor illnesses and injuries are treated in the clinic and the student is returned to the classroom. If a student becomes ill at school or sustains an injury requiring outside medical attention, the parent/guardian will be called. Please be sure to have your contact information updated in Infinite Campus at all times throughout the school year. 

Reporting Injuries: If your child is injured or comes to school with an ace wrap, splint, cast, sling or is on crutches, we will always ask for a note from the doctor regarding activity restrictions related to gym and recess. If your child is excused from gym class, he/she may not participate in recess. A doctor’s note is required for them to return to full participation in both gym and recess. 

Parties, Treats, & Snacks:  Please remember that food allergies are increasing in children every year. Nuts and nut byproducts are one of the biggest offenders among food allergies. Please be careful and make sure snacks brought into the classroom do not contain nuts.

Lake County Free Medical Clinic: Offers short-term, low cost medical and dental care to clients in Lake County. Serves adults and kids with no or low income, who do not have health insurance, state medical assistance, or are underinsured.

Lake County General Health District: This agency provides a multitude of services for residents of Lake County. Offers health information, and clinics for adults and children alike.

National Institute of Health: NIH is the nation’s medical research agency.

 

Needy Meds: Find help with the cost of medicine. Get prescription assistance. This is a drug discount program and not an insurance plan.
